FEES AND INSURANCE




FEES

Initial Phone Consultation  -  Free

Session Fee (60 minutes)  For those who have insurance  -  $100.00 (*see note below)
Session Fee (60 minutes)  Client pay or uninsured               -  $ 50.00

Phone sessions are billed at the same rate as office visits. 


Bounced Check Fees  -  $ 25.00

    Please verify your health insurance coverage when you arrange your first visit and I will verify your co-pay.

* All self pay clients are expected to take care of their fees at the time of service.
* Insured clients are expected to pay their portion (co-pay) at the time of service.
* Client's insurance company will be billed for any provided services and you will be
   billed for any balances due on your account. 
* Until your deductible is met, you will only be billed $50 per 1 hour session.  (*note)
